CommCare Skilled Nursing

HRV has partnered with a variety of trusted partners to finance the development of a new skilled nursing facility in rural Natchitoches, Louisiana. The new facility will be operated by CommCare Corporation, which has owned and operated skilled nursing and rehabilitation centers throughout Louisiana since 1994 and is currently the largest non-profit nursing home owner in Louisiana.  The $24 million development cost of the project is supported by Federal and State tax credit financing in which HRV joined with investments made by Chase Community Equity, U. S. Bancorp Community Development Corporation, IberiaBank, Advantage Capital, and AMCREF Community Capital.

The new facility is specifically designed to carry out CommCare’s person-centered care initiatives, involving transforming the atmosphere of the traditional long-term care center to direct choice, dignity, and purpose for the residents.  The project will provide vital nursing, therapy and pharmacy services to residents of a low income community, with 125 full-time positions anticipated in the new center.  Over 70% of the positions will be filled by minority residents of Natchitoches.  Also, 80% of the 112 patients to be served at the new 72,000 SF center are expected to be Medicare or Medicaid dependent.
